Check-in & Registration

When arriving to the clinic, you will check in with our receptionist and review/complete any registration paperwork. We will discuss your insurance and benefits with you prior to your evaluation. Next, you will be taken to an exam room for the initial interview portion of the evaluation.

Physical therapy Evaluation

First Visit - Ascent Physical Therapy & Performance

During the evaluation your physical therapist will begin by asking you a series of questions about your health and the specific condition for which you are seeking treatment. We like details! Information about you and your condition will help us determine what the cause of you pain or condition is, and which treatments are most likely to help you. Your therapist may ask you specific questions about your home or work environment, your health habits, activity level, and your leisure and recreational interests so that the therapist can help you become as active and independent as possible.

We want to get to the exact cause for why you are seeing us. To get these answers, we will perform a detailed examination. Depending on your symptoms and condition, we might examine your range of motion, strength, flexibility, balance, coordination, posture, movement, and measure vitals. Your physical therapist might use his or her hands to examine or "palpate" the affected area or to perform a detailed assessment of the mobility of your joints, muscles, and other tissues.

Your physical therapist might evaluate:

  • How you walk (your "gait")
  • How you get up from a lying position or get in and out of a chair ("functional activities")
  • How you use your body for certain activities, such as bending and lifting ("body mechanics")

Your Diagnosis

Based on all of the information we gather during the interview portion and the examination, we will develop a physical therapy diagnosis related to your medical condition. Once we determine your physical therapy diagnosis, we will discuss with you what your diagnosis is and why you are having your current symptoms. We can provide you with handouts or videos to help you better understand your diagnosis. The more you understand, often the better your outcome!

Your goals are our goals. We will work with you to determine your goals for physical therapy and will begin to develop a plan for your treatment.

The Best Treatment Plan

We will discuss your treatment options. We have many tools in our toolbox and want you to be comfortable. We will provide you with what we believe is the best treatment plan, but together we will determine the best course of action for you. We will provide all the tools necessary to achieve your and now our goals.

Your First Treatment

Next, we put the plan to action! We like to provide treatment on your first visit. As we have mentioned before, we want you feeling better as fast as possible! Based on your specific treatment plan, we will begin to implement the necessary physical therapy techniques and procedures to help you Restore, Move, and Perform your best.
